Online Registration Procedures



Registration Form

Please enter the information requested below. The name and mailing address of the person who is actually registering for the Internet course should be entered here. The required fields necessary to process your registration are indicated with an *. A working email address is required before you register. When you have successfully submitted your registration and credit card payment, you will receive an email from seafoodhaccp.cornell.edu within one to three days with instructions on how to obtain and activate a Cornell Guest ID Username and Password. After you complete this process you will receive instructions by email on how to go to the Seafood HACCP Course Site and Log in with the Username and Password you have created.

When you have completed the Internet course a “Notification of Course Completion” will be addressed to the person entered on this registration form and sent to the Email address entered below.

When you have finished filling out this form click on the “Submit Form” button below. You will then be asked to enter the credit card number that you'll be using for payment and your credit card billing information. This will be a secure credit card transaction.

Please note: We are unable to process transactions for credit cards issued by banks outside of the U.S. at this time. Please use the “registering by mail” option on the proceeding page.
